• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Evaluating and treating patients in SNF setting
• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborating with facility staff to ensure excellent care for your patients
• Re-evaluating patient progress, revising plans of care and resetting goals
• Completing timely and accurate documentation including evaluations, re-evaluations discharges and daily progress notes
• Supervising Occupational Therapy Assistants.
