Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our team in a SNF setting, providing patient-centered evaluation and treatment aligned with individualized plans of care.

You will evaluate patients, develop plans of care, set functional goals, document progress, and collaborate with facility staff. Supervising OTAs is included.

This role emphasizes hands-on therapy, achieving outcomes, and delivering excellent patient service to patients and families.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including evaluations, re-evaluations discharges and daily progress notes
• Supervising Occupational Therapy Assistants
• Collaborating with facility staff to ensure excellent care for patients
