Job summary

Enhance Rehabilitation is actively seeking an Occupational Therapist to join our team in a skilled nursing facility setting. The role involves evaluating and treating patients, developing individualized plans of care, and collaborating with facility staff to ensure high-quality patient outcomes.

You will perform re-evaluations, adjust goals, and educate patients and families about progress and expected outcomes while maintaining thorough documentation.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
