Job summary

Resilient Health in Gilbert, AZ is seeking a Case Manager to coordinate care for members with mental illness and/or addiction. You will assess needs, develop individualized plans and facilitate community and home-based services to support recovery.

You will maintain charting and ensure timely, compliant documentation, while coordinating with teams, families and providers to secure a wide range of services and supports.

Qualifications

• High School Diploma or equivalent with six months of behavioral health work experience or internship.
• Associates Degree with three months of behavioral health experience or internship.
• BHT Certificate, or Bachelor's/Master's Degree as applicable.

Responsibilities

• Participates in the assessment, evaluation and planning of patient care.
• Develops individualized service plans for participants.
• Provides community and home-based services to support treatment goals.
• Coordinates meetings, staffing, case conferences and other meetings as appropriate.
• Provides referrals to outside providers and assists in securing social services.
• Maintains documentation (charting) per agency and funding source requirements.
• Communicates with participants or families to coordinate consent, releases and referrals for care.
• Coordinates transportation to and from appointments when appropriate.
• Meets with service providers to review utilization and progress toward goals.
• Provides participant advocacy and education as needed.
• Develops transitional service plans prior to reentry into the community.

About our Case Manager Position:

The Case Manager is responsible for the delivery of quality patientcare to members via the coordination of a member's adult recovery teamin securing and managing general medical and behavioral health servicesthat aid in the member's recovery from mental illness and/or addiction.

What a Case Manager Does at Resilient Health:

• Participates in the assessment, evaluation and planning of patient care
• Develops individualized service plans for participants
• Provides community and home-based services to enhance and achieve treatment goals
• Develops, facilitates and coordinates meetings, staffing's, case conferences and other meetings as appropriate
• Providesreferrals to outside providers and assists in securing a variety ofsocial services, including, but not limited to, relapse preventionservices, medical care, and general assistance
• Maintains a minimum number of documented client contact hours per week
• Maintainsappropriate documentation (charting) and documents accurately andtimely in accordance with agency and funding source requirements
• Communicateswith the participant (or family member) to coordinate consent to care,releases and/or referrals for provider care which includes behavioralcare, general medical care, community resources, educational, social,judicial and other state agencies
• Coordinates transportation of participants to and from appointments when appropriate
• Consultswith treatment staff to coordinate effectiveness of services containedin the participant's individual service plans and make adjustments whereneeded
• Provides outreach and follow-up on crisis contacts and missed appointments
• Meets with service providers to review utilization and monitor member progress towards program goals
• Provides participant advocacy and education as needed, to ensure delivery of required services
• Develops transitional service plan (including discharge planning) with members, prior to reentry into the community

Requirements for this Position:

• Behavioral Health Technician (BHT) required (must be 21 years old):
  • HighSchool Diploma or equivalent and six months of full-time behavioralhealth work experience or internship or Peer Support Certification, or
  • Associates Degree and three months of full-time behavioral health experience or internship or Peer Support Certification, or
  • A BHT Certificate from an accredited college/university, or
  • Bachelor's Degree, or
  • Master's Degree
• Valid driver's license
• Valid CPR/First Aid certifications or ability to obtain within 30 days of hire
• Valid Fingerprint Clearance Card or ability to obtain within 90 days of hire
• Three-year Motor Vehicle Record that meets our auto insurance carrier's coverage requirements

What This Position Pays:

$20.39 - $22.62 per hour, depending on education and experience
