Job summary

Terros Health is seeking a dedicated Case Manager to join our interdisciplinary clinical team serving individuals with Serious Mental Illness or general mental health needs at our 51st Ave. Recovery Health Center in Phoenix, AZ.

This full-time, 40-hour role runs Monday–Friday, 8:30am–5:30pm, with comprehensive benefits and training for new grads. You will collaborate with clinicians to assess needs, develop service plans, coordinate with medical and behavioral health providers, and help clients

Qualifications

• No experience required with a bachelor's degree in behavioral health; training will be provided.
• New grads welcome.

Responsibilities

• Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals.
• Completes assessments, crisis plans and individualized service plans.
• Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented.
• Conducts outreach and follow-up of services including crisis interactions, missed appointments, and hospital discharges to ensure resources are available.
• Performs all case management functions including assessment and service planning processes and identifying needed evaluations.
• Pursues best practice outcomes for individuals with mental illness including continuing education, employment, independent housing and community tenure.

We are seeking a Case Manager to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our 51st Ave. Recovery Health Center.

Full-Time Employed: 40 hours/week, Monday to Friday 8:30am - 5:30pm

Additional Language Differential Pay Available; NOW OFFERING AN ADDITIONAL $1.50/hr SMI DIFFERENTIAL
