Job summary

Trinity Rehab, LLC seeks a dedicated Speech-Language Pathologist (SLP) for part-time home health in Suffolk, VA. You will evaluate and treat patients with speech, language, cognitive, and swallowing disorders under ASHA standards and physician oversight.

Responsibilities include conducting comprehensive evaluations, developing individualized plans, delivering direct therapy, and educating patients and caregivers. Strong communication and documentation skills are essential.

Qualifications

• Master’s degree in Speech-Language Pathology is required or equivalent.
• Active Virginia state license (or eligibility) is required.
• CCC-SLP and/or CF is preferred.

Position Summary

Trinity Rehab is seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to join our growing Home Health team in Suffolk, VA.

The SLP will evaluate, plan, and implement individualized treatment programs for patients experiencing speech, language, cognitive, and swallowing disorders. Services are provided in accordance with the standards set by the American Speech-Language-Hearing Association (ASHA) and under the direction of the attending physician.

Key Responsibilities

• Conduct comprehensive evaluations of speech, language, cognitive, and swallowing disorders
• Develop and implement individualized treatment plans
• Provide direct therapy services to patients in a home health setting
• Monitor patient progress and adjust treatment plans as needed
• Educate patients and caregivers on therapy techniques and home programs
• Maintain accurate and timely documentation
• Collaborate with interdisciplinary team members to ensure quality patient care
