Job summary

Sunnyvale Post Acute seeks a compassionate Speech-Language Pathologist (SLP) to join our rehabilitation team in Sunnyvale, CA. We offer Full-Time, Part-Time, and PRN opportunities to help residents improve communication, swallowing, and quality of life.

You will evaluate and treat residents with speech, language, cognitive, and swallowing needs, collaborating with nursing, physicians, and the interdisciplinary team. California SLP licensure is required; experience in post-acute care is preferred.

Qualifications

• Current California Speech-Language Pathologist (SLP) license.
• Appropriate certification and credentials required to practice in California.
• Experience in skilled nursing, post-acute care, or rehabilitation preferred.

Responsibilities

• Evaluate residents and develop individualized speech, language, cognitive, and swallowing treatment plans.
• Provide skilled therapy services based on resident needs and established care plans.
• Monitor resident progress and modify treatment approaches as appropriate.
• Complete accurate and timely clinical documentation.
• Collaborate with nursing, therapy, dietary, and other interdisciplinary team members.
• Educate residents, families, and caregivers regarding treatment recommendations and strategies.
• Promote resident safety, independence, communication, and quality of life.
• Maintain compliance with applicable professional, state, and federal standards.
