Flagstar Rehab in New York is seeking a Speech Language Pathologist to join our Skilled Nursing Facility team. You will perform evaluations, develop plans of care, and deliver effective therapy to patients with diverse diagnoses in a fast-paced SNF setting.

Qualifications

• Not accepting Clinical Fellows (CFY) at this time.
• Current NYS SLP License and Registration.
• CCC certification.
• CPR certificate.
• EMR competence.
• Meets all State and Federal requirements for US employment in a Nursing Facility.

Responsibilities

• Provide efficient speech or swallow evaluations and/or screens of patients.
• Develop plan of care, downgrade/upgrade as appropriate, manage meal texture.
• Provide therapy services to individuals in an SNF setting with diverse diagnoses.
