Job summary

Flagstar Rehab is seeking a Speech Language Pathologist (SLP) for skilled nursing facilities throughout NYC and surrounding areas. You will perform evaluations, develop plans of care, and provide therapy to patients with diverse conditions, while educating patients and families to improve daily functioning.

We offer flexible schedules (Full-Time, Part-Time, Per Diem), weekly pay, and benefits, with options for W-2 employment or independent contracting. CFY not accepted at this time.

Qualifications

• Not accepting Clinical Fellows (CFY) at this time.
• Current NYS SLP License and Registration.
• Clinical Competence Certification (CCC).
• CPR Certificate.
• EMR Competence.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Provide efficient speech or swallow evaluations and/or screens of patients.
• Develop plan of care, downgrade/upgrade as appropriate, manage meal texture.
• Provide quality therapy services to individuals in an SNF setting with diverse diagnoses.
• Monitor and document patient progress using EMR software.
