Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Paris, TN. This PRN position supports resident care by delivering speech therapy services, assessing communication disorders, and collaborating with the care team to meet regulatory standards.

Responsibilities include data collection for treatment planning, developing goals, instructing patients, families, and staff, and ensuring CMS-compliant documentation in NetHealth EMR.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC).
• Active SLP license in state(s) of practice.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data necessary for treatment planning.
• Develop a treatment plan that includes a target list, short-term and long-term goals.
• Instructs patient, family, and staff in appropriate speech-language procedures for carryover of remediation and goals.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversight and implementation of FEES/MBS protocols with site coordination.
• Participate in screening protocols to ensure timely meeting of resident needs with clinical excellence.

Education
