Job summary

Evolve Therapy Services is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility in Paris, TN. The PRN role focuses on planning, directing, and delivering Speech Language Pathology services in compliance with state and CMS standards.

Qualified candidates hold a Master's in Speech Language Pathology, CCC-SLP, and an active state license. Strong documentation skills in NetHealth EMR and ongoing professional development are expected.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency (CCC-SLP).
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan with targets, short-term and long-term goals.
• Instructs patient, family, and staff in speech-language procedures to allow carryover of therapeutic remediation and goals in daily activities.
• Complete compliant and medically necessary documentation per CMS expectations in NetHealth EMR.
• Oversee and implement proper FEES/MBS protocols with site coordination.
• Participate in screening protocols to ensure needs of residents are met timely and with clinical excellence.

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.

• To evaluate and obtain data necessary for treatment planning and implementation

• To develop a treatment plan that includes a target list, short-term, and long-term goals

• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities

• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs

• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications
