West Roxbury Health & Rehabilitation Center is seeking a compassionate Registered Speech-Language Pathologist (SLP) to provide individualized therapy for residents with communication, cognitive, swallowing, and functional needs. Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current Speech Language Pathology license in the applicable state.
• CCC or CFY eligibility.
• Long-term care experience preferred.
• Strong communication and interpersonal skills.
• Compassionate and resident-focused approach to care.
• Ability to develop creative, individualized treatment strategies.
• Strong organizational and documentation skills.
• Commitment to teamwork, collaboration, and quality outcomes.

Responsibilities

• Evaluate residents and develop individualized treatment plans based on their communication, cognitive, swallowing, and functional needs.
• Provide skilled speech therapy services in accordance with physician orders, professional standards, and regulatory requirements.
• Implement therapeutic interventions designed to improve communication, cognition, swallowing function, and independence.
• Complete timely assessments, screenings, progress notes, and clinical documentation.
• Monitor resident progress and adjust treatment approaches to support meaningful outcomes.
• Educate residents, families, and caregivers on strategies to support continued progress and success.
• Partner with nursing and interdisciplinary teams to support resident-centered care.
• Participate in care planning and discharge planning activities.
• Communicate effectively regarding resident progress, treatment goals, and recommendations.
