Job summary

Chestnut Park Rehabilitation & Nursing Center seeks a Speech Language Pathologist (CFY eligible) for a full-time role. You will evaluate residents, develop personalized treatment plans, select activities to improve daily independence, and monitor progress to adjust care plans. The CFY path offers mentorship and ongoing professional development.

Qualifications

• Education: Graduate of an accredited Speech Therapy Program.
• Licensing: Current New York State Speech Language Pathologist License.

Responsibilities

• Resident Evaluation & Treatment Planning: Assess residents' speech and language needs and develop personalized treatment plans.
• Activity Selection: Design activities that help residents improve independence in their daily activities.
• Progress Monitoring: Observe and assess resident performance, adjusting treatments as necessary, and preparing detailed progress reports.
• Rehabilitation Support: Assist in the rehabilitation of residents affected by injury, illness, or other conditions.
• Maintain a Safe Environment: Ensure a clean, safe, and supportive environment for residents and staff.
• Confidentiality: Maintain resident confidence by ensuring the secure handling of confidential information.
• Documentation: Keep accurate and up-to-date records of resident care and progress, auditing department records as necessary.
• Team Collaboration: Work closely with healthcare teams to contribute to a cooperative and supportive team environment.
