Job summary

Responsibilities

• Perform weekly head-to-toe skin assessments and document in the EHR.
• Implement treatments or interventions for identified skin issues.
• Collaborate with Clinician or Wound NP for orders as needed.
• Update ADON and DON on ulcers or non-healing wounds.
• Maintain resident care plans and integrity of wound documentation.
• Participate in SIT meetings and promote infection control practices.
