Job summary

Sanctuary Pointe Nursing & Rehab Center in Cincinnati, OH is seeking a dedicated Wound Care Nurse to lead and support our wound prevention and treatment programs.

The role requires nursing licensure in Ohio (RN or LPN), CPR, and at least 1 year of wound care experience, with 3+ years in healthcare preferred. You will assess, treat, and coordinate care for residents.

Qualifications

• Graduate of an accredited School of Nursing.
• Current LPN or RN license in the State of Ohio.
• Current CPR certification.
• Minimum of 1 year of experience in wound care required.
• At least 3 years of experience in healthcare or long-term care preferred.

Responsibilities

• Assess, treat, and monitor residents with acute and chronic wounds.
• Develop and implement individualized wound care plans and prevention strategies.
• Conduct routine rounds to evaluate risk factors and ensure appropriate interventions.
• Collaborate with physicians and interdisciplinary teams to coordinate care and treatment plans.
• Educate and guide nursing staff on wound care protocols, prevention techniques, and best practices.
• Monitor and ensure compliance with all wound care plans and regulatory requirements.
• Track and report wound care data, including pressure ulcers and treatment outcomes.
• Evaluate new admissions and existing residents for pressure ulcer risk and implement preventative measures.
• Ensure proper supply levels of wound care materials and equipment.
• Participate in quality improvement initiatives and support regulatory surveys.
• Provide education and emotional support to residents and families.
