Qualifications

• Discharge planning experience in a skilled nursing facility.
• Bachelor's or master's in social work or related field.
• Familiarity with MDS, care planning, Medicaid/Medicare a plus.
• Able to communicate effectively with residents, families, and staff.
• Organized with the ability to manage a busy caseload.
• LMSW/LCSW licensure welcomed but not required.
• Bilingual abilities are a bonus.

Responsibilities

• Plan safe discharges for short-term rehab patients with families and services.
• Complete psychosocial assessments and MDS sections on time.
• Participate in care plan and interdisciplinary team meetings.
• Assist new residents and families with admission and adjustment.
• Advocate for residents’ rights and emotional well-being.
• Help with Medicaid applications and community resources.
• Assist with advance directives and MOLST conversations.
• Maintain accurate, timely, and survey-ready documentation.
