Taylor County Health & Rehab is seeking an Admissions Coordinator to manage pre-admission and admission processes, ensuring a smooth patient transition and consistent experience. You will collaborate with families, referral sources, and central intake to complete paperwork and verify readiness of rooms for admission.

Qualifications

• Bachelor’s degree in Business Administration, Social Work, Marketing or related field.
• Associate’s degree with three years experience.

Responsabilités

• Coordinate pre-admission and admission processes and complete paperwork with family and patient.
• Maintain contact with patients and families to support transitions.
• Provide frequent visits to new admissions for a positive patient experience.
• Orient the patient to the center environment.
• Participate in the center’s IDT to assist in healthcare navigation needs.
• Develop partnerships with affiliated healthcare partners and navigators.
• Oversee outreach plans for nights and weekends admissions.
• Promote the image and reputation of the System through servant leadership.
