Job summary

Reliant Medical Group, part of the Optum family, seeks a 0.6 FTE Advanced Practice Clinician (NP/PA) to join our Skilled Nursing Facility team in Worcester, MA. The role supports value-based care and collaborates with physicians to align with goals of care in a SNF setting.

Located in Central Massachusetts, you’ll provide care on skilled rehab and long-term units, lead a multidisciplinary team, and manage common elderly-related conditions while following state guidelines and maintaining

Qualifications

National Board Certification is required.
Licensed by the Commonwealth of Massachusetts as an NP.
Graduate of an Accredited Nurse Practitioner Program.
Maintains BLS certification and any professional certifications as appropriate.

Responsibilities

Provide value-based care to patients on skilled rehab and long-term care units.
Collaborate with a physician to manage patients based on goals of care (recovery, palliation, or comfort).
Lead a multidisciplinary skilled rehab team (nursing, dietary, social work, PT, OT, SLP, and case management).
Monitor and manage acute and chronic conditions common in nursing homes (e.g., CVA, dementia, Parkinson’s, MS, fractures, delirium, post-op care).
Provide longitudinal follow-up and care for long-term care residents per state regulations.
Discuss goals of care with patients, families, and caregivers.
