Reliant Medical Group, part of the Optum family, seeks a 0.6 FTE Advanced Practice Clinician (NP/PA) to join our Skilled Nursing Facility team in Worcester, MA. You will provide care on skilled rehab and long-term care units, partnering with physicians and leading a multidisciplinary team to achieve goals of care.

Qualifikationen

• National Board Certification required.
• Licensed by the Commonwealth of Massachusetts Board of Registration of Nursing.
• Graduate of an Accredited Nurse Practitioner Program.
• Maintains BLS certification, professional certification as appropriate.

Aufgaben

• Provide value-based care to patients on skilled rehab and long-term care units.
• Partner with a physician to manage patients based on goals of care (recovery, palliative, or comfort care).
• Lead multidisciplinary skilled rehab team including nursing, dietary, social work, PT, OT, SLP, and case management.
