Job summary

Optum seeks an Advanced Practice Clinician (NP/PA) in Leominster, MA to deliver value-based care to SNF residents across rehab, long-term and palliative populations. Work with physicians and interdisciplinary teams to manage complex geriatric conditions.

Qualifications

• APC with MA licensure or ability to obtain; NP or PA credentialing required.
• National Board Certification and BLS certification required.
• Minimum 2 years APC experience preferred.

Advanced Practice Clinician (NP/PA) - Skilled Nursing Facility | Leominster, MA

Join Reliant Medical Group, part of Optum, as a Nurse Practitioner or Physician Assistant providing value-based care to residents in a skilled nursing facility setting. This role focuses on caring for older adults across skilled rehabilitation, long-term care, and palliative care populations while working closely with physicians and interdisciplinary care teams.

• 0.6 FTE (24 hours/week), Monday-Friday schedule

The salary range for this role is $109,500 to $164,000 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
