Advanced Practice Clinician (NP/PA) - Skilled Nursing Facility | Leominster, MA

Join Reliant Medical Group, part of Optum, as a Nurse Practitioner or Physician Assistant providing value-based care to residents in a skilled nursing facility setting. This role focuses on caring for older adults across skilled rehabilitation, long-term care, and palliative care populations while working closely with physicians and interdisciplinary care teams.

• Skilled Nursing Facility position in the Leominster, MA area
• 0.6 FTE (24 hours/week), Monday-Friday schedule
• Care for skilled rehab, long-term care, and palliative care patients
• Collaborate with physician partners and multidisciplinary teams

• Participate in goals-of-care discussions with patients and families
• Geriatric, skilled nursing, or palliative care experience preferred
• Minimum 2 years of APC experience preferred

The salary range for this role is $109,500 to $164,000 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
