The role requires strong relationship-building, excellent communication, and the ability to manage referrals and cross-functional coordination to improve admission conversions while delivering high-quality resident experiences.

Qualifications

• Experience in skilled nursing or post-acute admissions.
• Strong knowledge of Medicare/Medicaid and payer processes.
• Excellent communication and relationship-building abilities.
• Ability to manage multiple referrals in a fast-paced environment.

Responsibilities

• Manage admissions from referral through admission and coordination with nursing and clinical teams.
• Review referrals for appropriateness and coordinate with referral sources.
• Ensure timely communication of admissions decisions and processes.
• Coordinate with departments to ensure smooth admissions and transitions.
• Develop strategies to increase census and referrals with high-value partners.
• Maintain accurate records of referrals, admissions, denials, and conversion activity.
