Job summary

Guardian Angel Home Care seeks a compassionate Licensed Practical Nurse (LPN) to join our home health team in the Grand Rapids area. You will provide skilled nursing care to patients in their homes under the supervision of an RN and physician.

We value autonomy, flexible scheduling, and strong patient relationships. The ideal candidate has an active Michigan LPN license, is proficient in documentation, and committed to high-quality, compliant care in a home setting.

Qualifications

• Active Michigan Licensed Practical Nurse (LPN) license in good standing.
• Graduate of an accredited practical nursing program.
• Current BLS/CPR Certification
• Valid driver's license, reliable transportation, and current auto insurance
• Strong clinical, communication, and documentation skills
• Ability to work independently in a home health setting

Responsibilities

• Provide skilled nursing care in patients' homes under the direction of the Registered Nurse and physician
• Monitor patient condition, vital signs, and overall health status
• Administer medications and treatments as ordered and within the LPN scope of practice
• Accurately document all visits and care provided in Homecare Homebase (HCHB)
• Communicate changes in patient condition to the supervising RN and physician
• Educate patients and families on medications, disease management, and the plan of care
• Ensure compliance with all federal, state, and agency regulations
• Collaborate with the interdisciplinary team to achieve optimal patient outcomes
