Job summary

Guardian Angel Home Care is seeking a compassionate Licensed Practical Nurse (LPN) to provide high-quality nursing care in patients’ homes. The role offers autonomy, flexible scheduling, and meaningful patient relationships within a supportive interdisciplinary team.

You will monitor patients, administer medications, document care in HCHB, and collaborate with RNs and physicians to ensure optimal outcomes. LPN program completion and BLS/CPR are required.

Qualifications

• Graduate of an accredited practical nursing program.
• Proficient documentation and patient assessment skills.
• Ability to work independently in a home health setting.

Responsibilities

• Provide skilled nursing care in patients' homes under the direction of the RN and physician.
• Monitor patient condition, vital signs, and overall health status.
• Administer medications and treatments as ordered within the LPN scope.
• Accurately document visits and care in HCHB.
• Communicate changes in patient condition to the supervising RN and physician.
• Educate patients and families on medications and plan of care.
• Ensure compliance with federal, state, and agency regulations.
• Collaborate with the interdisciplinary team for optimal outcomes.

Guardian Angel Home Care is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our growing home health team. In this role, you will provide high-quality, patient-centered care in the comfort of patients' homes while working alongside a supportive interdisciplinary team.

Responsibilities

• Provide skilled nursing care in patients' homes under the direction of the Registered Nurse and physician
• Monitor patient condition, vital signs, and overall health status
• Administer medications and treatments as ordered and within the LPN scope of practice
• Accurately document all visits and care provided in Homecare Homebase (HCHB)
• Communicate changes in patient condition to the supervising RN and physician
• Educate patients and families on medications, disease management, and the plan of care
• Ensure compliance with all federal, state, and agency regulations
• Collaborate with the interdisciplinary team to achieve optimal patient outcomes

Requirements

• Graduate of an accredited practical nursing program
• Current BLS/CPR Certification
• Valid driver's license, reliable transportation, and current auto insurance
• Strong clinical, communication, and documentation skills
• Ability to work independently in a home health setting

Preferred Qualifications

• Home health or hospice experience
• Experience with Homecare Homebase (HCHB) or other electronic medical record systems
• Wound care experience
• Bilingual candidates are encouraged to apply
