This role offers autonomy, flexible scheduling, and meaningful patient relationships. You will monitor conditions, administer treatments, document visits in HCHB, and educate patients and families on care plans.

Qualifications

• Active LPN license in good standing (Michigan).
• Graduate of an accredited practical nursing program.
• BLS/CPR certification current.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing care in patients' homes under RN/physician direction.
• Monitor patient condition, vital signs, and health status.
• Administer medications and treatments as ordered within LPN scope.
• Document visits and care provided in Homecare Homebase (HCHB).
• Communicate changes in patient condition to supervising RN/physician.
• Educate patients and families on medications, disease management, and care plan.
• Ensure compliance with federal, state, and agency regulations.
• Collaborate with interdisciplinary team to improve patient outcomes.

Responsibilities

• Provide skilled nursing care in patients' homes under the direction of the Registered Nurse and physician
• Monitor patient condition, vital signs, and overall health status
• Administer medications and treatments as ordered and within the LPN scope of practice
• Accurately document all visits and care provided in Homecare Homebase (HCHB)
• Communicate changes in patient condition to the supervising RN and physician
• Educate patients and families on medications, disease management, and the plan of care
• Ensure compliance with all federal, state, and agency regulations

Requirements

• Active Michigan Licensed Practical Nurse (LPN) license in good standing

• Strong clinical, communication, and documentation skills
• Ability to work independently in a home health setting

Preferred Qualifications

• Home health or hospice experience
• Experience with Homecare Homebase (HCHB) or other electronic medical record systems
• Wound care experience
