Guardian Angel Home Care is seeking a compassionate Licensed Practical Nurse (LPN) to join our home health team in the Grand Rapids area. You will provide patient-centered nursing care in patients' homes, working with a supportive interdisciplinary team.

Qualifications

• Active Michigan LPN license in good standing.
• Current BLS/CPR certification.
• Valid driver’s license and reliable transportation.
• Strong clinical, communication, and documentation skills.
• Ability to work independently in a home health setting.

Responsibilities

• Provide skilled nursing care in patients' homes under the direction of the RN and physician.
• Monitor patient condition, vital signs, and overall health status.
• Administer medications and treatments as ordered within the LPN scope of practice.
• Document visits and care provided in the EMR.
• Communicate changes in patient condition to the supervising RN and physician.
• Educate patients and families on medications, disease management, and plan of care.
• Ensure compliance with federal, state, and agency regulations.
• Collaborate with the interdisciplinary team to achieve optimal patient outcomes.

Requirements
