Guardian Angel Home Care Inc. is seeking a compassionate Licensed Practical Nurse (LPN) to join our home health team in the Grand Rapids area. You will provide skilled nursing care in patients' homes under RN and physician direction, maintain thorough documentation, and educate patients and families on care plans.

Formación

• Active Michigan LPN license in good standing.
• Graduate of an accredited practical nursing program.
• Current BLS/CPR Certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong clinical, communication, and documentation skills.
• Ability to work independently in a home health setting.

Responsabilidades

• Provide skilled nursing care in patients' homes under the direction of the Registered Nurse and physician.
• Monitor patient condition, vital signs, and overall health status.
• Administer medications and treatments as ordered and within the LPN scope of practice.
• Accurately document all visits and care provided in Homecare Homebase (HCHB).
• Communicate changes in patient condition to the supervising RN and physician.
• Educate patients and families on medications, disease management, and the plan of care.
• Ensure compliance with all federal, state, and agency regulations.
• Collaborate with the interdisciplinary team to achieve optimal patient outcomes.
