Job summary

Guardian Angel Home Care is seeking a compassionate Licensed Practical Nurse (LPN) to join our Home Health team in Akron. The LPN will provide one-on-one skilled nursing care in patients' homes, support independence, and deliver care with autonomy and collaboration with a clinical team.

This role requires an active Ohio LPN license, BLS/CPR, strong communication and documentation skills, and the ability to work independently.

Qualifications

• Active Ohio LPN license in good standing.
• Graduate of an accredited practical nursing program.
• Current BLS/CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong communication, organizational, and documentation skills.
• Ability to work independently in a home health environment.

Responsibilities

• Provide skilled nursing care in patients' homes
• Assess and monitor patient conditions
• Administer medications and treatments as ordered
• Perform wound care and other skilled nursing procedures
• Educate patients and caregivers on medications, disease processes, and care plans
• Document all patient visits accurately and timely using HCHB
• Communicate changes in patient condition to the RN Case Manager and physician
• Collaborate with the interdisciplinary care team to ensure excellent patient outcomes
• Maintain compliance with agency, state, and federal regulations

Preferred Qualifications

• Previous home health or hospice experience
• Experience with Homecare Homebase (HCHB) or another EMR
