Meridian Health Services in Indiana is seeking a full-time Skill Builder I for the Children\'s Program in Madison County. You’ll work with therapists and medical staff to coordinate care and implement individualized treatment plans.

As a Skill Builder, you’ll deliver life skills training, support families, complete assessments, and participate in ongoing professional development in a collaborative, mission-driven team focused on whole-person care.

Qualificações

• High school diploma or equivalent with 2 years behavioral healthcare experience or equivalent college coursework toward a human services or health related degree.
• Associate Degree preferred.

Responsabilidades

• Collaborate with therapists to develop, implement, and revise treatment plans.
• Deliver individual and group skills training for adults and children.
• Focus on self-care and daily life management.
• Focus on interpersonal communication and parenting.
• Focus on illness and anger management.
• Focus on positive coping and problem-solving skills.
• Provide case management services aligned with treatment goals.
• Coordinate with medical providers and identify health-related needs.
• Complete CANS/ANSA reassessments and maintain documentation.
• Serve as a liaison between patients and external agencies.
• Participate in ongoing professional development and training.
