Job summary

Meridian Health Services is hiring a full-time, day-shift Skill Builder I for the Children's Program in Madison County, Indiana. You will work with therapists and medical staff to coordinate care and implement individualized treatment plans, providing life skills training to adults and children to support developmental milestones.

Requirements include a high school diploma or equivalent with 2 years in behavioral healthcare (or related coursework); an Associate Degree is preferred.

Qualifications

• High school diploma or equivalent with 2 years behavioral healthcare experience or equivalent coursework toward a human services or health related degree.
• Associate Degree preferred.

Responsibilities

• Collaborate with therapists to develop, implement, and revise treatment plans.
• Deliver individual and group skills training for adults and children, focusing on self-care, daily life management, interpersonal communication, parenting, illness and anger management, and coping skills.
• Provide case management services aligned with treatment goals.
• Support integrated care by coordinating with medical providers and identifying health-related needs.
• Complete CANS/ANSA reassessments and maintain accurate clinical documentation.
• Serve as a liaison between patients and external agencies or institutions.
• Participate in ongoing professional development and training.
