Job summary

Meridian Health Services is seeking a full-time Skill Builder I for our Children's program located in Madison County, IN. The role provides skills and development training to assist patients and/or families in achieving developmental milestones and reducing psychosocial barriers.

The position involves case management in coordination with the clinical team, using professional judgment to implement the treatment plan and support Integrated care.

Qualifications

• High school diploma or its equivalent with 2 years paid behavioral healthcare experience or equivalent college coursework toward a human services or health related degree required at minimum
• Valid driver's license and reliable transportation may be required
• Commitment to providing individual attention to help clients achieve mental, emotional and physical well-being

Responsibilities

• Assists in developing, implementing and revising individual treatment plans and monitors progress toward goals
• Provides skills training and development to patients and/or families for adults and children as part of the treatment plan
• Coordinates with Therapists and medical staff to ensure integrated care and effective communication
• Completes assessments and maintains clinical records and reports
• Participates in continuing education activities and maintains professional knowledge
