Qualifications

• Prior experience in case management with individuals who have severe mental illnesses preferred.
• Valid Ohio driver's license and car required.
• Flexible work schedule, including evenings and weekends.

Responsibilities

• Monitor and coordinate evaluations to identify individual needs.
• Assist in developing Interdisciplinary Service Plans.
• Help individuals manage psychiatric symptoms and develop coping skills.
• Provide crisis intervention and support as needed.
• Document progress and participate in staff meetings.
