Job summary

Community Support Services is seeking an AOD Community Rehabilitation Specialist (CRS) to join the Site Based Supportive Housing team in Akron, OH. The CRS coordinates services, facilitates ISP planning, and helps clients access community resources to support recovery and independence.

Working with Case Managers, Recovery Specialists, and medical staff, you’ll monitor evaluations, assist with daily living skills, and ensure occupancy rules are upheld while maintaining thorough documentation.

Qualifications

  • Bachelor’s degree in a human services field.
  • Experience in case management with substance abuse clients.
  • Valid Ohio driver’s license and insurability.
  • Chemical Dependency license preferred.
  • Knowledge of Housing First philosophy.
  • Strong organizational and communication skills.
  • Ability to work in a team environment.

Responsibilities

  • Coordinate and monitor evaluations and assessments to identify needs.
  • Assist individuals to identify and minimize effects of AOD/psychiatric symptoms.
  • Develop and reassess Interdisciplinary Service Plans (ISP).
  • Connect clients with community resources and build coping skills.
  • Accompany clients to appointments and assist in daily living skills.
  • Ensure residents comply with occupancy rules by the housing program.
  • Maintain relevant documentation.
  • Attend team meetings and collaborate with medical staff.

The Opportunity: We are seeking to hire an AOD Community Rehabilitation Specialist (CRS) to join one of our case management team at our Site Based Supportive Housing location. This individual provides ongoing recovery focused community rehabilitation services to individuals with AOD issues and/or severe and persistent mental illnesses. The CRS is a service broker, coordinator, monitor, and planner of services, offering practical help, building community support, teaching recovery-based skills and providing active outreach, advocacy, and crisis assistance based on the strengths, needs, abilities, and preferences of the individual.What You’ll Do:Serve on a team built up of fellow Case Managers, Recovery Specialists, and medical staff to navigate the needs and care of your clients, attending team meetings on a regular basis.Monitor and coordinate the delivery of necessary evaluations and assessments in order to identify each person’s needs.Assist individuals to identify and minimize the effects of the AOD and/or psychiatric symptoms, enabling individuals to self-manage their illness.Coordinate and participate in the development of an Interdisciplinary Service Plan (ISP) that shall include services and assistance necessary to achieve specific objectives.In cooperation with the individual, re-assess strengths, needs, abilities and expressed preferences as required by the “ISP” standards specified in Rule 5122-27-04 of the Administrative Code, and/or at the request of the individual.Connect clients with community resources to help them develop interpersonal and community coping skills.Assist the individual in fostering and increasing their social support networks of relatives, AA or NA community, Dry Clubs, friends, and/or volunteers.Accompany individuals to appointments and activities, assisting in daily living skills training as needed.Assist the Site Based Supportive Housing location in ensuring residents are compliant with occupancy rules established by the National Church Residence and Akron Metropolitan Housing Authority.Maintain relevant documentation.This position is full-time (40 hours a week) working normal business hours.About Us: For over 35 years, Community Support Services has been making a difference in the lives of countless individuals with severe and persistent mental illnesses.
