Cura of Willmar is seeking a compassionate Short-Term Rehabilitation Social Worker to support residents and families throughout admission to discharge. You will provide counseling, resources, and advocacy while collaborating with an interdisciplinary team.

The role emphasizes discharge planning, post-acute coordination, and ensuring compliant documentation within a quality care framework.

Qualifications

• Bachelor's degree in Social Work required.
• Minnesota Social Work licensure preferred.
• Previous experience in skilled nursing, rehabilitation, healthcare, or case management preferred.
• Excellent communication, organization, and problem-solving skills.

Responsibilities

• Meet residents and families to provide orientation and answer questions.
• Complete psychosocial assessments and document findings in the medical record.
• Provide emotional support, counseling, and resource referrals as appropriate.
• Coordinate discharge planning beginning at admission and ensure safe plans.
• Maintain accurate documentation and comply with HIPAA and regulations.
