Job summary

Apple Rehab is seeking a Social Work Assistant to implement and document care plans addressing residents’ social and emotional needs. This role collaborates with nursing, MDS, admissions and rehabilitation to facilitate discharge planning and uphold residents’ rights.

The position requires a Bachelor’s degree in Human Services or related field (preferred Social Work). A vehicle and valid license are needed; excellent teamwork and communication are essential for resident and family support.

Qualifications

• Bachelor’s Degree in Human Services or related field.
• Knowledge of admissions and discharge procedures and residents’ rights.
• Attention to detail and ability to prioritize multiple tasks.
• Innovative and creative approach to serving resident needs with a customer focus.
• Must have a vehicle and a valid drivers’ license.
• Demonstrates cooperation and ability to work as a team.

Responsibilities

• Craft and document personalized care plans for residents' social and emotional well-being.
• Collaborate with nursing, MDS, Admissions and rehab to facilitate discharge plans.
• Inform and educate staff, residents and families about residents' rights.
• Maintain updated resources for residents and families in the community.
• Address reported concerns, provide resolutions, and ensure proper documentation.
• Provide access to residents, families and staff for support and guidance.
• Offer timely referrals to appropriate agencies for assistance.
