Job summary

Cura of Willmar is seeking a compassionate Resident & Family Services Social Worker to support residents and families in a skilled nursing setting. The role focuses on psychosocial support, advocacy, and coordinating services, four days per week on-site.

You will collaborate with the interdisciplinary team to improve quality of life, conduct assessments, assist with care planning, and ensure clear communication, confidentiality, and compliance with regulations.

Qualifications

• Bachelor's degree in Social Work is preferred.
• Minnesota Social Work licensure is preferred.
• Experience in long-term care, healthcare, case management, or customer service is preferred.

Responsibilities

• Provide psychosocial support to residents and families.
• Advocate for resident rights, preferences, dignity, and independence.
• Complete psychosocial assessments and required social service documentation.
• Collaborate with the interdisciplinary team to enhance resident experience.
• Maintain accurate documentation and ensure HIPAA compliance.
