Job summary

Telecare seeks a Case Manager III to provide primary support and case management for individuals with mental illness, helping them live independently in their community. The role requires education and experience with co-occurring disorders and operates under direct supervision.

Shifts vary, typically Monday–Friday, 8:30 AM–5:00 PM. Starting pay range is $23.00–$27.63 per hour, with geographic differentials applied. Compensation based on experience and location.

Qualifications

• Bachelor’s degree in Social Services or Human Services and 1 year of experience or High School Graduation/GED plus four years of direct services in Mental Health or community services.
• Valid and current driver’s license and personal vehicle insurance.
• Willingness to drive clients using personal vehicle (IRS mileage reimbursement).
• Willingness to use company vehicle for client transport.

What You Will Do to Change Lives

Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Case Manager III is distinguished from Case Manager II by the additional education, years of experience, and ability to proficiently perform the job functions and meet position standards.

Shifts Available:

Shifts very as needed, Monday-Friday 8:30 AM- 5:00 PM

Expected starting wage range is $23.00 - $27.63.

Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)

• Bachelor’s degree in Social Services or Human Services
  and (1) year of experience or High School Graduation or G.E.D. equivalent
  and four (4) years of direct services in Mental Health or community services;
  an Associate's degree may be substituted for two (2) year of experience.


• Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.


• Willingness to use your personal vehicle to drive clients to appointments and groups, etc.(weekly mileage reimbursement at the IRS rate)


• Willingness to use the company vehicle to drive clients to appointments and groups, etc.
