Job summary

BXFN is seeking a Case Manager to support residents in a DHS shelter in NYC. You will conduct needs assessments, develop ILPs, and provide ongoing case management and crisis intervention.

Qualifications

• Bachelor’s degree in a relevant field from an accredited institution.
• Minimum 2 years of Case Management experience.
• Experience with homeless or underserved populations.
• Familiarity with community resources, housing, and benefits.
• Proficiency in Microsoft Office and CARES case management software.
• Excellent oral and written communication, organizational, and time management skills.
• Bilingual English/Spanish preferred.

Responsibilities

• Conduct assessments to determine client needs and develop ILPs with ongoing case management.
• Conduct weekly/bi-weekly ILPs with clients; document meetings in CARES within 48 hours.
• Perform weekly/bi-weekly unit inspections and document in CARES.
• Check and respond to work emails daily to ensure timely communication.
• Assist clients with housing search, viewings, job search, and PA case opening.
• Maintain written and oral communication of incidents per CARES policies.
• Ensure accurate documentation of intake forms, progress notes, and records.
• Connect clients with housing, healthcare, and social services; advocate for needs.
• Educate clients on budgeting, landlord-tenant relations, and healthy living.
• Adhere to shelter policies, DHS and OTDA regulations, and reporting requirements.
• Collaborate with shelter staff and service providers for coordinated care.
• Manage caseload of 20–25 cases with up-to-date records.
• Assist clients with medical/human services referrals in the community.
• Perform general admin tasks, including calls, emails, reports, and service meals.
• Report issues to the shelter director; perform other duties as assigned.
