Project Renewal in New York City is seeking a Case Manager to support residents in Bronx, Brooklyn, and Manhattan shelters. You will conduct intake and psychosocial assessments, help clients access housing and entitlements, and develop Independent Living Plans with concrete steps toward stability.
Collaborate with on-site health teams, document accurately, and guide clients toward self-sufficiency through counseling and referrals, while complying with agency policies and relevant regulations.
