Project Renewal in New York is seeking a Case Manager for CanopyWomen’s Shelter. You will conduct intake, assessments, and direct services, develop Independent Living Plans, and document thoroughly to ensure audit readiness.

You will assist clients in securing housing and entitlements, coordinate with staff and external providers, and support women facing homelessness and health challenges toward greater independence and permanent housing.

Qualifications

  • Bachelor’s degree preferred.

  • High School Diploma or GED with 2 years of relevant experience.

Responsibilities

  • Conduct intake and assessments and complete psychosocial assessments.
  • Provide case management and develop Independent Living Plans with clients.
  • Enter client data into CARES system and maintain charts.
  • Coordinate with services to obtain housing, entitlements, and other resources.
  • Maintain up-to-date documentation per guidelines; audit-ready.
  • Guide clients in goals, housing, sobriety, meds, and employment.
  • Liaise with community organizations and attend program meetings.
  • Provide escorts to appointments as needed.
  • Perform other duties as assigned.

Program Description:

Canopy House provides 171 emergency shelter beds for homeless women diagnosed with mental illness and/or substance use disorders. Our comprehensive services include case management, individual and group counseling, recreational activities, occupational therapy, mental health services and housing placement assistance. Canopy House is a unique facility in that it is co-located with a supportive housing program that occupies the top 15 floors of the building and offers a street facing Article 28 clinic on the first floor dedicated to providing primary and behavioral health care services.


Positions Description:

Under the supervision of the Director of Social Services and the Team Leader, the Case Manager conducts required assessments and provides case management and direct services to clients. In this role, the Case Manager supports clients in addressing identified needs and building the skills and stability necessary to regain control of their lives, achieve greater independence, and transition to permanent housing or other appropriate community-based housing options. The Case Manager also provides referrals and links clients to appropriate internal and external resources, assists with other cases as needed, and ensures all required protocols are followed. The Case Manager maintains client charts in accordance with program standards and ensures documentation remains complete, accurate, and audit-ready at all times, and performs other related duties as assigned.


Essential Duties and Responsibilities:

  • Conducts intake and assessments, makes necessary referrals for services, and completes/updates psycho-social assessments of clients
  • Provides case management services and assists clients in the creation of and adherence to their Independent Living Plans by identifying short and long-term objectives that can be achieved
  • Inputs all client information and updates in a timely fashion into the CARES system as well as the client’s physical chart
  • Works collaboratively with clients and other staff to identify and obtain financial entitlements, housing, and other concrete services
  • Maintains accurate and up-to-date written client documentation as required and in accordance with City/State oversight agency guidelines
  • Guides clients through individualized sessions on issues such as achieving personal goals, finding permanent or alternative housing, maintaining sobriety, complying with medication protocols, and pursuing employment opportunities
  • Maintains liaison with representatives from community organizations
  • Provides escort services to clients’ appointments as needed
  • Attends and participates in the program meetings
  • Performs other duties as assigned by the Team Leader, Director of Social Services, and/or Program Director

Physical Activities:

While performing the duties of this position, the Case Manager must be able to navigate the 6-story facility by foot, walking up and down the stairwells, to observe staff and clients in the event the elevator is out of service. The Case Manager must be able to stand and bend to review client files, put them away, and close them out. It is also part of the role of sitting at the computer workstation for extended periods to review the client data and update the information in the case management systems. The Case Manager must be able to lift or move material and equipment weighing up to 15 pounds.


Qualifications:

  • Bachelor’s degree from an accredited college or university is preferred.
  • An Associate’s degree with one (1) year of experience working with homeless, mentally ill and/or chemically addicted individuals or other special needs populations.
  • A High School Diploma or GED equivalent with two (2) years of experience working with homeless, mentally ill and/or chemically addicted individuals or other special needs populations.
  • Bilingual or multilingual a plus

Preferred Skills:

  • Team Player: Ability to work cooperatively with peers and other staff in order to serve the needs of the clients as effectively and efficiently as possible.
  • Communication: Excellent oral, writing, and listening skills are a must.
  • Organizational: The ability to work well within a highly pressurized, deadline environment, while meeting the short-term and long-term mandates of the program.
  • Interpersonal: An ability to interface with clients as well as all levels of staff.
  • Computer: Knowledge of case management software as well as proficiency in Microsoft Office Suite
