Qualifications

  • Fully licensed LMSW in the state of practice.
  • Master's degree in Social Work.
  • Minimum 1 year of clinical social work experience.
  • Spanish or other languages spoken by communities served is a plus.

Responsibilities

  • Develop relationships with primary care teams to assess social needs.
  • Plan and implement support packages to address socioeconomic and medical barriers.
  • Navigate managed care plans for community services.
  • Casemanage social and behavioral care for self-management.
  • Document patient encounters and sessions accurately in clinical systems.
  • Identify ways to improve ArchWell Health's approach based on feedback.
  • Support advance care transitions with members and families.
  • Assist access to state prescription programs and other benefits.

Duties/Responsibilities:
  • Develop relationships and collaborate with the primary care teams to conduct social services assessments to determine the appropriate needs for each member
  • Plan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriers
  • Navigatemanagedcareplansforcommunityservicesandprograms
  • Casemanagementforsocialandbehavioralcaretoallowmemberstoself-managehealthand socialservicesupport
  • Accurate and timely documentation of patient encounters and sessions in all clinical management systems
  • Proactivelyidentify methods to improve ArchWellHealth's approach based on feedback and regularly conducted surveys
  • Support advance care transitions with members and their families
  • Assist members with access to state-based prescription programs and other benefits
RequiredSkills/Abilities:
  • Experience supporting patients with need for social services
  • Experience with screening, assessment, and planning for common social services needs
  • Working knowledge of differential diagnosis of common mental health conditions
  • Strong interpersonal communication skills with exceptional active listening abilities
  • Highly empathetic, non-judgmental, and open-minded
  • Experience in a collaborative team environment
Education and Experience:
  • Fully licensed LMSW in the desired State of practice
  • Master's degree in Social Work
  • 1+years' experience in clinical social work
  • A problem-solving orientation and a flexible and positive attitude
  • Missiondrivenandmotivatedtojoinanorganizationthatwilltransformthewaywedeliver accessible,clinicallyexcellentcaretoseniors
  • Proficient PC skills
  • Fluency in Spanish or other languages spoken by people in the communities we serve (where necessary)
