Job summary

ArchWell Health in Missouri is seeking a Clinical Social Worker (MSW) to join our team serving seniors in community clinics. You will collaborate with primary care teams to assess social needs, plan supportive packages, and help members overcome socioeconomic and medical barriers.

The ideal candidate is LMSW licensed in the state, has at least 1 year of clinical social work experience, fluent in Spanish or other languages used by our communities, and demonstrates empathy, strong communication,

Qualifications

• Fully licensed LMSW in the desired State of practice.
• Master's degree in Social Work.
• 1+ years' experience in clinical social work.
• Mission-driven and adaptable attitude.
• Proficient PC skills.
• Fluency in Spanish or other languages spoken by communities served where necessary.

Responsibilities

• Develop relationships and collaborate with the primary care teams to conduct social services assessments to determine the appropriate needs for each member.
• Plan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriers.
• Navigate managed care plans for community services and programs.
• Casemanagement for social and behavioral care to allow members to self-manage health and social service support.
• Accurate and timely documentation of patient encounters and sessions in all clinical management systems.
• Proactively identify methods to improve ArchWell Health's approach based on feedback and regularly conducted surveys.
• Support advance care transitions with members and their families.
• Assist members with access to state-based prescription programs and other benefits.

Education

Master's degree in Social Work
