Job summary

Integrated Life Choices seeks a Shared Living Provider (Independent Contractor) to support individuals with developmental disabilities in their home in North Platte, NE. The role includes maintaining a safe, healthy living environment, providing meals, transportation to appointments and day services, and supporting access to community activities in line with each Individual Support Plan (ISP).

Responsibilities include daily documentation, attending meetings, and ensuring compliance with agency

Qualifications

• Must have valid driver license, car insurance, and current vehicle registration.
• Pass a State/Federal and APS/CPS background check.
• Experience supporting an individual with developmental disabilities.
• Completion of all required trainings.
• Must have detailed verbal and written communication and computer skills.
• Must understand and comply with agency and state policy and procedures.
• Must attend and participate in all meetings.
• Must maintain a clean and orderly home.
• Must have a high school diploma or GED.
• Must be at least 19 years of age.
• Must be able to lift 50 pounds, kneel, twist, reach, crawl, run, etc.

Responsibilities

• Provide residential supports in the individual’s home as outlined in the ISP.
• Maintain a safe and healthy home environment.
• Provide nutritional meals and transportation to appointments and day services.
• Support access to community activities, cultural and religious services as appropriate.
• Provide up to 24-hour supervision and supports as outlined in the ISP.
• Daily documentation of teaching and activities.
