Responsibilities

• Provide personal care and assistance with ADLs (bathing, dressing, grooming, toileting).
• Observe and respond to residents’ needs while promoting independence.
• Maintain confidentiality and privacy of resident information and documentation.
• Communicate effectively with coworkers and interdisciplinary teams.
• Participate in CST meetings to discuss resident progress and needs.
• Document and report changes in health, behavior, or nutrition.
• Follow infection control and safety protocols.
• Assist with safe transfers, positioning, and mobility support.
• Support restorative nursing and rehab programs as per plan.
• Measure and record vital signs, weight, and height.
• Assist with meals and fluids according to dietary plans.

We are currently hiring a Shahbaz to join our Nursing team and provide comprehensive, person-centered care in a home-like environment that fosters dignity, autonomy, and quality of life for residents.

Wage: $22.50-$24.86/hr. DOE Schedule: Part-Time NOC, 10:30pm-6:30am (45 hrs. ) 0.5 FTE

What You’ll Do

• Provide personal care and assistance with activities of daily living (ADLs), including bathing, dressing, grooming, oral hygiene, and toileting
• Observe and respond to elders’ needs, promoting independence and decision-making
• Maintain confidentiality and privacy of all elder care and documentation
• Communicate effectively with coworkers and interdisciplinary teams to meet resident needs
• Participate in Clinical Service Team (CST) meetings to discuss resident progress and needs
• Document and report changes in health, behavior, eating habits, skin condition, and psychosocial status
• Follow protocols for infection control, fire safety, disaster preparedness, and facility policies
• Assist with safe transfers, positioning, and mobility support using appropriate equipment and techniques
• Support restorative nursing and rehabilitation programs as outlined in care plans
• Measure and document vital signs, weight, and height
• Assist with meals, snacks, and fluids based on dietary plans and preferences
• Maintain cleanliness and organization within the household, including resident rooms, shared spaces, and kitchen areas
• Complete laundry tasks for residents including washing, drying, and folding
• Follow safety protocols and report unsafe conditions or suspected abuse immediately
• Participate in ongoing training and education to maintain compliance with state and federal regulations
