Job summary

Mosaic is seeking a Service Support Specialist to help empower people through life-changing services and strong community partnerships. You will bridge between community partners and the services Mosaic provides, ensuring client needs are met with care and accuracy.

Ideal candidates have a high school diploma, at least one year in service coordination, and some supervisory exposure. CPR/First Aid and related certifications are valued alongside solid software skills and clear communication.

Qualifications

• High school diploma or equivalent required.
• One year of experience in service coordination or delivery preferred.
• Two years of supervisory experience (or equivalent college studies) preferred.
• Certifications: CPR/First Aid, Article 9, and Principles of Caregiver.
• Strong proficiency with business software and professional communication; manage complex documents and deadlines.

Responsibilities

• Coordinating Care: Managing new referrals and intake assessments to ensure clients receive high-quality care.
• Building Relationships: Fostering connections with Managed Care Plans, Case Managers, and community partners.
• Supporting Our Workforce: Assisting with onboarding and coordination of Direct Care Workers from hire to assignment.
• Ensuring Excellence: Conducting home visits and reviewing service plans to maintain high standards of care and compliance.
• Problem Solving: Managing scheduling changes and researching incidents or grievances for continuous service improvement.
