Education level: Bachelor's degree (B.A.) from a four year college or university and/or LPN license.
Responsible for managing a caseload of clients in an adult day program setting and ensuring that all required assessments, documentation, and service elements including but not limited to progress notes, assessments, data collection, plan updates, incident/behavior reports, summaries, and billing-related documentation are completed in accordance with all regulatory and program standards. This role requires consistent communication with clients, families, and staff, as well as ongoing collaboration with the AD Manager and the Interdisciplinary Team to monitor goal progress, respond to individual client needs, and develop person-centered Individual Program Plans for each client on the assigned caseload.
