ConnectAbility of MN is hiring a Service Coordinator to efficiently manage specialized equipment purchasing and authorized services for clients. The role centers on clear communication with case managers, clients, and vendors, ensuring services adhere to DHS waiver rules and approved Service Agreements.

Responsibilities include coordinating purchases, reviewing vendor quotes, and maintaining Salesforce records while collaborating with internal teams to ensure timely, compliant service delivery.

Qualifications

• High school diploma or equivalent.
• Strong verbal and written communication skills.
• Ability to manage multiple tasks, deadlines, and priorities in a fast-paced environment.
• Ability to interpret Service Agreements and follow DHS waiver rules.
• Comfort working in Salesforce, Excel, and SharePoint (training provided).
• Strong attention to detail and documentation accuracy.
• Ability to work independently while collaborating with internal teams.
• Must pass a background check.

Responsibilities

• Coordinate services that have been referred and authorized through approved Service Agreements.
• Interpret Service Agreements to start, stop, adjust, or hold services per DHS waiver rules.
• Monitor service status and thresholds and communicate changes to case managers, clients, and vendors.
• Ensure services align with authorization, timelines, and compliance requirements.
• Collaborate with Billing to resolve denied claims and ensure documentation is correct.
• Maintain accurate client records in Salesforce and run routine reports.
