Presbyterian SeniorCare Network in Pittsburgh, PA is seeking a Service Coordinator/Social Worker to connect residents with community providers and monitor ongoing services addressing social determinants of health.

The role involves referrals, coordination across agencies, and ensuring effective support for residents within our SeniorCare Network housing options. Full-time, daytime hours with competitive benefits.

Qualifications

• Have a bachelor's degree (Social Work, Gerontology or related field a plus!).
• Have a valid Pennsylvania driver's license and access to a registered and insured vehicle for travel between communities.
• Able to effectively interact with residents and team members.

Responsibilities

• Establish ties to service providers and agencies in the community.
• Refer residents to providers and monitor the provision of services.
• Monitor services addressing social determinants of health and bridge gaps between communities, individuals and providers.

Starting Pay: $21.00/hour - $26.00/hour; Depending on experience

The Service Coordinator/Social Worker is responsible for establishing ties to service providers and agencies in the community, referring residents to such providers and monitoring the provision of these services. The Service Coordinator is responsible for the monitoring of services that address the social determinants of health and bridge gaps between communities, individuals and providers.
