Catholic Charities of the Archdiocese of Washington is seeking a Clinical Care Coordinator to assess consumers and lead individualized rehabilitation plans under the Recovery Model. The role focuses on ACT teams, IRP development, and coordinated care across hospitals and community settings.

You will work with the ACT team to ensure timely assessments, treatment planning, and effective transitions, while maintaining compliance with DC social work standards and agency policies.

Qualifications

Bachelor’s degree in Social Work or related field.
Current DC professional license as applicable i.e. LSWA.
Three years of experience working with consumers in mental health setting.

Responsibilities

Provide services to consumers in accordance with the ACT Fidelity Model.
Lead development, updating, and implementation of Individual Recovery Plans (IRP) with the team.
Coordinate treatment planning and discharge meetings at area hospitals for smooth transitions.
Maintain professional clinical relationships with hospital staff for seamless care transitions.
Educate consumers on learning daily living activities and health benefit access.
Update IRP, FPSA, LOCUS, GAINS and Crisis Plan every 180 days.
Provide an average of 4.25 hours of services per day to ACT consumers.
Conduct psychosocial assessments every 180 days for ACT clients.
Develop and implement quality improvement activities for clinical staff and ACT team.

JOB SUMMARY:

The Clinical Care Coordinator provides assessment of consumers’ functioning and leads development and documentation of individualized rehabilitation plans based on the Recovery Model for adults who have mental illnesses.

ESSENTIAL DUTIES And RESPONSIBILITIES

Provide services to consumers in accordance with the principles and practices of the ACT Fidelity Model.
Lead the process of developing, updating, and guiding implementation of consumers’ Individual Recovery Plan (IRP), in collaboration with consumer, Team Leader and other significant parties of the ACT team.
Coordinate treatment planning and discharge meetings at area hospitals to ensure a smooth transition to the community.
Develop and maintain a professional and clinical relationship with area hospital staff to ensure a smooth transition for agency consumers to and from the hospital.
Educate and assist consumers with learning or re-learning activities of daily living (ADL) skills necessary to improve the quality of life including acquisition of consumer health care benefits.
Update Consumer’s IRP, Functional Psychosocial Assessment (FPSA), Level of Care Utilization System (LOCUS), Global Appraisal of Individual Needs (GAINS) and Crisis Plan every 180 days.
Provide an average of 4.25 hours of services per day to ACT consumers.
Establish constructive relationships with consumers to promote acquisition of rehabilitative goals.
Maintain adherence to all policies and procedures consistent with Dept. of Mental health (DMH) and the District of Columbia board of social work.
Participate in treatment team meetings and conduct follows ups as indicated in the following areas:
Crisis assessment and intervention.
Symptom assessment, management and individual supportive therapy.
Assistance in the treatment of substance abuse for consumers with co-occurring addictive disorders.
Psychosocial rehabilitation and skill development.
Recommend and sometimes make decisions about the best course of action for a particular service user.
Provide support services to ACT team and consumers.
Develop IRP and other care plans as indicated.
Conduct needs and risk assessment.
Provide resolution and referrals services for consumers.
Conduct psychosocial assessments using the Functional Psychosocial Assessment (FPSA) every 180 days for consumers served by the ACT team.
Education, support and consultation to consumers’ families and/or their support system, which is directed exclusively to the well-being and benefit of the consumer.
Develop and implement quality improvement (QI) activities for individual clinical staff members and the clinical aspects of the ACT team. Attend QI meetings as appropriate;
Ensure that all IRP’s, FPSA’s, and other pertinent paperwork is submitted on time.
Perform other job-related duties as assigned.

Requirements

EDUCATION and EXPERIENCE:

Bachelor’s degree in Social Work or related field.
Current DC professional license as applicable i.e. LSWA.
Three years of experience working with consumers in mental health setting.

SKILLS And COMPETENCIES

Driving is required. Must have valid local driver’s license and clean driving record. At least 40% of the time is spent in the community providing services to consumers in their natural settings; 50% of services are face-to-face.
Skill in the use of computer systems (preferably in a PC, Windows-based operating environment) and MS Office products, and able to become proficient in Catholic Charities’ electronic health record, database, and remote access systems.
