Crestwood Behavioral Health, Inc. is seeking a Service Coordinator (Mental Health Case Manager) in California. The role involves assessing clients to identify treatment needs and coordinating the interdisciplinary team for effective discharge planning to support recovery goals.

This full-time PM position offers opportunities to collaborate with nursing staff and to implement comprehensive treatment plans, ensuring continuity of care in a patient-centered, recovery-oriented environment.

Qualifications

• Two years of full-time experience in a mental health setting or equivalent part-time experience.
• Bachelor’s degree in psychology, social work, or behavioral sciences may substitute for two years of experience.
• Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
• Familiarity with regulatory standards.

Responsibilities

• Assess clients to identify problem areas necessitating treatment and complete assessments.
• Coordinate interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team.
• Coordinate the overall treatment plan implementation for the client in collaboration with nursing staff to ensure continuity of care.

Title: Service Coordinator (Mental Health Case Manager)

The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Mental Health Case Manager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. In this capacity, the Service Coordinator (Mental Health Case Manager) coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.

Qualifications

• Two (2) years of full-time experience or part-time equivalent in a mental health setting.
• A bachelor’s degree with a major in psychology, social work, or behavioral sciences may be substituted for the two (2) years of full-time work experience or its part-time equivalent.
